This notice explains how Maxi Care Plus collects, uses and protects personal information provided through our website, enquiries, surveys and referral pathways.

Who we are
Maxi Care Plus provides disability supports and is an approved registered NDIS provider. This notice applies to personal information handled through this website and our service enquiries. More detailed notices or consents may apply when you become a client, worker or supplier.
What we may collect
Depending on how you contact us, we may collect your name, contact details, relationship to a participant, NDIS and plan information, support needs, goals, communication preferences, feedback and other information you choose to provide. Referral and service information may include health or disability information, which is sensitive information under Australian privacy law.
How we collect and use it
We collect information directly from you, from an authorised representative or referrer, and through our website, telephone, email, surveys and secure PromaxCare portal. We use it to respond to enquiries, assess and arrange supports, communicate with you, manage feedback and complaints, meet safety and legal obligations, and improve our services. We seek consent where required, particularly for sensitive information.
Monthly surveys and website experiences
The Monthly Quality & Experience Survey is an internal quality, audit and service-improvement record. We do not transfer monthly survey answers into website testimonials. The separate Share an Experience form lets a person submit the exact words they approve for publication and choose how their identity may appear. Nothing is published automatically: an authorised Maxi Care Plus administrator must still review and approve the record.
When information may be shared
We may share information with staff and contractors who need it for their work, technology and professional service providers, government or regulatory bodies where required, and other people or organisations you authorise. We do not sell personal information. Some technology providers may process or store information outside Australia; where this occurs, we take reasonable steps required by law and can provide further information on request.
Storage and security
We use administrative, technical and physical safeguards appropriate to the information we hold. No online system is completely risk-free, so please do not send unnecessary sensitive information by ordinary email. Use the secure referral portal or call us if you are unsure.
Access, correction and privacy concerns
You may ask to access or correct personal information we hold about you, or raise a privacy concern, by emailing info@maxicareplus.com.au, calling 1300 648 114, or writing to Level 2, 11–17 Swanson Court, Belconnen ACT 2617. We may need to verify your identity. If you are not satisfied with our response, you may contact the Office of the Australian Information Commissioner.

This site links to external services, including the PromaxCare portal and government websites. Their privacy practices may differ from ours. We may update this notice when our services, systems or legal obligations change.
